This 33-year-old male was admitted for a high fever, abdominal pain, and a noted moderate decrease in alertness. Robert has been on oral antibiotics for a left ear infection for approximately 7 days. I last saw the patient in the office four days ago when his left ear spontaneously drained.

PE: Ht: 5' 11", Wt: 194, T: 101.2, R: 19, BP: 134/86. Pt seems confused. Left ear drainage continues.

CSF analysis reveals normal pressure, a slightly thicker viscosity with a cloudy appearance. Results of CSF exam showed 6875 WBC, [tex]35 \, \text{g/L}[/tex] protein, and [tex]23 \, \text{mg/dL}[/tex] glucose.

Arbovirus is identified in two blood cultures. Pt responds positively to antimicrobial therapy.

DX: Meningitis due to arbovirus, urban yellow fever, acute suppurative otitis media.
